Capital Health Network

Capital Health Network is the ACT’s primary health network.

Our role is to advance the way health care is delivered in Canberra. We address community needs by collaborating with consumers, GPs, clinicians, and sector stakeholders to improve health outcomes.

We are unique in our ability to support primary health care professionals and design services that fill gaps and deliver lasting improvements.

Care finder program

A new care finder program has commenced to support vulnerable older Canberrans needing intensive assistance to access aged care and other supports.

Care finder is designed to support older Canberrans who:

  • don’t have a support person and who are eligible for government-funded aged care

  • are 65 years or older

  • are 50 years or older and are Aboriginal and/ or Torres Strait Islander

  • are homeless or at risk of homelessness and on a low income

Care finders can help:

  • people understand what aged care services are available

  • set up and attend an assessment

  • complete forms to access services

  • older people to access other supports in the community if required, such as health, mental health, housing and homelessness, drug and alcohol services, and community groups.
